मानव की मधुमेह पर विजय की अमर गाथा

प्राचीन काल में हमारे महान आयुर्वेद शास्त्री सुस्रुत ने मधुमेह के बारे में बहुत कुछ लिखा है। 1500 वर्ष ईसा पूर्व पापायरस ने मधुमेह के उपचार के कई नुस्के बताये थे। अरेटियस ने दूसरी शताब्दी में मधुमेह के बारे में बताया था कि इस रोग में शरीर का मांस गलकर मूत्र द्वारा निकल जाता है और अन्ततः रोगी की मृत्यु हो जाती है।

उन्नीसवीं शताब्दी में यह देखा गया कि डायबिटीज से मरने वाले रोगियों का पेन्क्रियास अक्सर क्षतिग्रस्त होता था। 1869 में युवा चिकित्सक पॉल लेंगरहेम्स ने पाया कि पेन्क्रियास , जिसका मुख्य कार्य पाचन रस बनाना है, में कुछ विशिष्ट तरह की कोशिकाओं के झुंड होते हैं। ये पूरे पेन्क्रियास में फैले होते हैं और इनका कार्य अभी तक किसी को मालूम नहीं था। कालांतर में हुई खोज से स्पष्ट हो गया था कि कोशिकाओं के इस झुंड में कुछ कोशिकाएं, जिन्हें बीटा कोशिकाएं कहा गया, इन्सुलिन का स्राव करती हैं। कोशिकाओं के इस झुंड को “आइलेट्स ऑफ लेंगरहेम्स" नाम दिया गया क्योंकि इस झुंड को पहली बार इन्होंने ही पहचाना था।

सन् 1889 में अपने परीक्षणों से फिजियोलोजिस्ट मिनकोस्की और चिकित्सक वोन मेरिंग ने यह सिद्ध कर दिया था कि यदि कुत्ते का पेन्क्रियास निकाल दिया जाये तो कुत्ते को डायबिटीज हो जाती है। लेकिन यदि कुत्ते के पेन्क्रियास की नलिका, जिसके द्वारा पाचन रस आंत तक पहुंचता है, को बांध दिया जाये तो कुत्ते को थोड़ी पाचन संबम्धी तकलीफ जरूर होती है पर डायबिटीज नहीं होती है। इससे उन्होंने यह निष्कर्ष निकाला कि शरीर में पेन्क्रियास के कम से कम दो कार्य हैं। एक पाचन रस बनाना और दूसरा कोई अज्ञात तत्व बनाना जो रक्त शर्करा को नियंत्रित करता है। यदि इस अज्ञात तत्व को पहचान लिया जाये तो डायबिटीज के सारे रहस्य परत दर परत खुलते चले जायेंगे।

परंतु उन्नीसवीं शताब्दी के आखिर तक डायबिटीज एक गम्भीर और लाइलाज महामारी मानी जाती रही। इसके कारणों का पता लगाने के लिये विश्वभर में वैज्ञानिक और चिकित्सक शोध कर रहे थे, पर कहीं से भी कोई अच्छे परिणाम सामने नहीं आ रहे थे। तब 14 नवंबर, 1891 को कनाडा में एलिस्टन, एन्तारियो के मेहनती और अमीर कृषक परिवार में एक महान सितारा पैदा हुआ, जिनका नाम था फ्रेडरिक बेंटिंग। उनका परिवार उत्तरी आयरलैंड से आकर एन्तारियो में बसा था। बेंटिंग लिखायी पढ़ाई में ज्यादा तेज-तर्रार नहीं थे और मुश्किल से ही हाई स्कूल पास कर सके थे। फिर बैंटिग ने विक्टोरिया कालेज के आर्टस संकाय में प्रवेश लिया और पहली छमाही परीक्षा में ही जनाब फेल हो गये।

परिवार वालों ने उन्हें समझाया, बुझाया, सांत्वना दी और अपने प्रभाव से टोरोन्टो यूनिवर्सिटी के मेडीसिन स्कूल में दाखिला दिलवा दिया। लेकिन मेडिकल स्कूल ने उसे हिदायत दी कि उन्हें आर्टस स्कूल में जाकर पुनः परीक्षा पास करने पर ही यह प्रवेश मान्य होगा। मेडिकल स्कूल में भी वह कभी अच्छे अंक प्राप्त नहीं कर सके। तभी प्रथम विश्व युद्ध शुरू हो गया और वे कनाडा की सेना में भर्ती होना चाहते थे। वे साक्षातकार देने भी गये पर दो बार तो उनकी नजर खराब होने के कारण उन्हें वापस भेज दिया गया। तीसरी बार उन्होंने फिर प्रयास किया, इस बार वे सेना में भर्ती होने में सफल हो ही गये। इसके साथ उन्होंने अपना अध्ययन भी जारी रखा। मेडीसिन स्कूल से पास होते ही वह सेना में मेडीकल ऑफिसर के पद के लिए चुन लिए गये। 1917 में उनको सेना के एक खास मिशन पर विदेश भेजा गया जिसमें उन्होंने अपनी बहादुरी का परिचय दिया। रोगियों की सेवा करते करते एक बार वे घायल भी हो गये थे। उनकी बहादुरी के लिए उन्हें “मिलेट्री क्रॉस" पुरस्कार दिया गया।

1919 उन्होंने सेना की नौकरी छोड़ दी और लन्दन, एन्तारियो में मेडिकल प्रेक्टिस शुरू कर दी। यहां उन्होंने अपनी पुरानी मित्र एडिथ से सगाई भी की। इसके साथ ही उन्होंने पश्चिमी एन्तारियो के विश्वविद्यालय में विख्याता का पद ले लिया और प्रोफेसर विल्सन की रिसर्च टीम में भी शामिल हो गये। लेकिन वे अच्छा व्याख्यान नहीं दे पाये। उन्हीं दिनों एक बार उन्होंने मेडिकल जर्नल में मोसेज बेरोन का लेख पढ़ा, जिसमें यह परिकल्पना की गई थी कि यदि मिनकोस्की के प्रयोगों को आगे बढ़ाया जाये तो पेन्क्रियास से निकलने वाले स्राव को अलग किया जा सकता है, जिससे मधुमेह का इलाज किया जा सके।

बेंटिग इस लेख से बहुत प्रभावित हुए। उन्होंने इस विषय पर काफी चिंतन किया और यह निष्कर्ष निकाला कि पेनक्रियाज में बनने वाले पाचन रस हो न हो “आइलेट्स ऑफ लेंगरहेम्स" में बनने वाले अज्ञात स्राव को नष्ट कर देते हों। इसलिए वे पेनक्रियाज की नलिका को बांध देना चाहते थे, इससे पेनक्रियाज क्षतिग्रस्त होगी, सिकुड़ जायेगी और पाचन रस बनाना बंद कर देगी। फिर इस पेनक्रियाज से अज्ञात स्राव को निकाल लिया जायेगा जिससे डायबिटीज के रोगियों का इलाज किया जा सकता है। प्रोफेसर विल्सन ने बेंटिग को टोरोंटो विश्वविद्यालय में फिजियोलोजी के प्रोफेसर जे.जे.आर. मेक्लियोड से मिलने की सलाह दी और कहा कि वे उन्हें परीक्षण करने के लिए प्रयोगशाला उपलब्ध करवा लकते हैं।

बेंटिग ने प्रोफेसर मेक्लियोड को अपनी परिकल्पना बताई। पहले तो वे बेंटिग पर भड़के और फिर बोले, "क्या तुम पागल हुए हो? क्या तुम्हें मालूम नहीं कि बड़े बड़े महारथी सूरमां बरसों से इस विषय पर रिसर्च कर रहे हैं और आज तक किसी को कुछ हासिल नहीं हुआ है तो तुम जैसा अनुभहीन व्यक्ति कौन से तीर मार लेगा।” परंतु बेंटिग ने हार न मानी, मेक्लियोड को मनाने का हर संभव यत्न किया, खूब मिन्नते की। वे माने तो जरूर पर झुंझला कर बोले, " ठीक मैं तुम्हें दो महीने के लिए एक पुरानी प्रयोगशाला और परीक्षण करने के लिए कुछ कुत्ते दे देता हूं। पर याद रहे ठीक दो महीने बाद तुम्हें प्रयोगशाला खाली करनी पड़ेगी। (वे थोड़ा रुके और बेंटिंग को ऊपर से नीचे तक देख फिर बोले) पर तुम्हें रसायनशास्त्र का ज्यादा ज्ञान भी नहीं है, मैं कौशिश करता हूं कि तुम्हारी सहायता के लिए एक रसायनशास्त्री को नियुक्त कर दूं।”

27 फरवरी, 1899 को वेस्ट पेम्ब्रोक, वाशिंगटन काउन्टी, मायने में चार्स हरबर्ट बेस्ट नाम का एक और महान सितारा पैदा हुआ। इनके पिता डॉक्टर थे। बेस्ट टोरोंटो विश्वविद्यालय में फिजियोलोजी और बायोकेमिस्ट्री में ग्रेजुएशन कर रहे थे। जब वे फाइनल इयर में थे तब उन्हें और नोबेल को प्रोफेसर मेक्लियोड ने डायबिटीज की खोज हेतु एक चिकित्सक के साथ कार्य करने के लिए बुलाया। मेक्लियोड को दोनों में से एक को चुनना था पर दोनों ही यह कार्य करना चाहते थे। इसलिए यह निर्णय हुआ कि सिक्का उछाला जायेगा। टॉस भाग्यशाली बेस्ट ने जीता। यह चिकित्सक और कोई नहीं बेंटिग ही थे। दोनों युवा चिकित्सकों को प्रयोगशाला देकर मेक्लियोड छुट्टियां मनाने स्कॉटलैंड के लिए रवाना हो गये।

इस तरह बेंटिंग और बेस्ट मिले और कुत्तों पर अपने परीक्षण करने की रूपरेखा बनाने लगे। वे दोनों पक्के दोस्त बन गये थे। दोनों में उत्साह की कोई कमी नहीं थी। ये 1921 की गर्मियों के दिन थे। रोज सुबह दोनों उस छोटी सी, पुरानी लेब में पहुंच जाते। लेब ऐसी कि थी कि उसमें ठीक से सूरज का प्रकाश भी नहीं आता था, कई वर्षों से उसका रंग रोगन भी नहीं हुआ था। लेब में एक बैंच पड़ी थी और एक लकड़ी की पुरानी मेज पर कुछ शीशियां रखी हुई थी। कुत्तों का कमरा ऊपर था। कुत्तों के कमरे के बगल में एक बदबूदार छोटी सी कोठरी थी जिसमें कुछ औजार और बेकार फटे पुराने चिथड़े रखे थे। उनके पास पैसों और साधनों की भारी कमी थी। कुत्तों को सिखाने, नहलाने और उनकी ब्लड शुगर करने के लिए एक सहायक रखने के भी पैसे नहीं थे। तब किसको मालूम था कि इस पुरानी, गंदी, बदबूदार और बेकार पड़ी लेब में एक जानलेवा और लाइलाज बीमारी डायबिटीज का उपचार की खोज होने जा रही थी।

सबसे पहले उन्होंने कुछ कुत्तों का ऑपरेशन करके उनका पेनक्रियाज निकाल लिया। ऑपरेशन के बाद कुत्तों की ब्लड शुगर बढ़ने लगी और डायबिटीज के लक्षण दिखने लगे यानी उन्हें डायबिटीज हो गई। दूसरे परीक्षण में उन्होंने कुछ कुत्तों के पेन्क्रियास की डक्ट को बांध दिया। जिससे पेन्क्रियास का पोषण बंद हो गया और वह सिकुड़ने लगा। 5-6 हफ्ते बाद उन्होने दूसरे कुत्ते का पेन्क्रियास निकाला और उसके छोटे छोटे टुकड़े करके नमक के घोल में डाल कर फ्रीजर में जमने के लिए रख दिया। आधा जमने पर पेन्क्रियास को पीसा और छान कर उसका रस निकाल लिया। इसे उन्होंने "आइजलेटिन” का नाम दिया।

अब आई शनिवार 30 जुलाई, 1921 की वो एतिहासिक सुबह, इन्सुलिन के खोज की घड़ी। उन्होंने उस रस का 4 एम.एल. कुत्ते नं. 391 से 410 को इंजेक्ट किया। कुत्तों का ब्लड शुगर 0.20 से 0.12 प्रतिशत कम हो गया। उन्होंने दो घंटे बाद कुत्तों को फिर इंजेक्ट किया। कुत्ते में डायबिटीज के लक्षण ठीक होने लगे। चिकित्सा जगत में एक बड़ी खोज हो चुकी थी। यह दिन इतिहास के पन्नों में सुनहरी स्याही से लिखा जा चुका था। उन्होंने पूरी रिपोर्ट प्रोफेसर मेक्लियोड को तार द्वारा भेजी। तार पढ़ कर मेक्लियोड अचंभित थे। वे जान चुके थे कि एक महान खोज हो चुकी है। उसे यह बात हजम नहीं हो रही थी कि आज के दो अनाड़ी लड़के इतनी महान खोज कर चुके थे। वे यह सोचते हुए तुरंत वापस लौटे कि कैसे इस विजय का सेहरा अपने माथे पर बांधें।

मेक्लियोड ने आते ही ऐसा जाल बिछाना शुरू कर दिया कि खोज का पूरा श्रेय उन्हीं को मिले। उन्होंने "आइजलेटिन” का नाम बदल कर "इनसुलिन” रख दिया। मेक्लियोड ने उन्हें अच्छी लेब, पर्याप्त धन और सभी संभव संसाधन मुहैया करवाए। उनकी मदद के लिए अपने एक बायोकेमिस्ट्री के स्कोलर कोलिप को भी उनकी टीम में शामिल कर दिया। कोलिप को इंसुलिन को शोधन करने का कार्य सौंपा गया। उसने इस पूरे प्रकरण की जानकारी टोरोंटो विश्वविद्यालय को दे दी।

उनकी खोज की खबरें चिकित्सा जगत में चर्चित होने लगी थी। तभी मई 1922 में वाशिंगटन में हुई एक मेडिकल कान्फ्रेन्स में पूरी रिसर्च के बारे में विस्तार से चर्चा हुई। अब बारी थी इंसुलिन के मानव परीक्षणों की। उनके द्वारा परिष्कृत किये गये इन्सुलिन सबसे पहले लियोनार्ड थोमस को दिया गया जो टोरेन्टो हॉस्पिटल में डायबिटीज के कारण बेहोशी की हालत में भर्ती करवाया गया था। पहली बार उसे इन्सुलिन का इन्जेक्शन दिया गया जिससे उसे चमत्कारी लाभ हुआ और कुछ ही दिनों में पूर्णतया स्वस्थ होकर वह घर लौट गया।

अब बेंटिंग और बेस्ट के लिये टोरेन्टो विश्वविद्यालय परिसर में एक शोध केन्द्र स्थापित किया गया, जिसका नाम “बेंटिंग और बेस्ट रिसर्च सेन्टर" रखा गया। अभी भी बहुत काम होना बाकी था इन्सुलिन को परिष्कृत करने की तकनीक विकसित करनी थी।

लेकिन तभी नोबेल प्राइज कमेटी ने मेडीकल और फिजियोलोजी में नोबेल प्राइज के लिये बेंटिग और मेक्लियोर्ड का नाम प्रसारित हुआ। 1923 में यह सुनकर बेंटिग को बहुत बुरा लगा। इस बात पर बहुत वाद-विवाद हुआ और नोबेल प्राइज कमेटी की बहुत छीछालेदर हुई। कई बार नोबेल प्राइज कमेटी अपने पक्षपातपूर्ण रवैये के लिए विवादों में रही है। बेंटिंग ने कहा कि यदि बेस्ट को नोबल प्राइज नहीं मिलता है तो वे भी नोबेल प्राइज स्वीकार नहीं करेंगे। पर उनके मित्रों और सहयोगियो ने उसे समझाया कि पहली बार किसी कनाडा के नागरिक को नोबेल प्राइज मिलने जा रहा है, उनकी महान खोज को दुनिया के हर डायबिटीज रोगी को पहुंचाने का वास्ता दिया गया तब बड़ी मुश्किल से वे नोबेल पुरस्कार लेने के लिये तैयार हुऐ लेकिन अपनी नोबेल प्राइज की राशि का आधा भाग अपने बेस्ट मित्र को दे दिया।

मेक्लियॉड ने भी आधी इनाम की आधी राशि कोलिप को सौंपी। बेंटिग के उनकी प्रेमिका एडिथ से अच्छे संबंध नहीं रहे और वे शादी भी नहीं हो सकी। तब उनकी जिंदगी में आई टोरेन्टो की रईस धनाड्य स्त्री मेरियोन रोबिन्सन, जिससे उन्होंने विवाह किया, परंतु यह विवाह ज्यादा नहीं चला और 1929 में उनका विवाह-विच्छेद हो गया। फिर कुछ वर्षों बाद उन्हें हेनरिता बोर्न नाम की सुन्दर स्त्री से प्यार हुआ, जिससे उन्होंने 1939 में विवाह रचाया।

द्वितीय विश्व युद्ध में बैंटिग पुनः कनाड़ा की सेना में भर्ती हो गये और 1941 में मेडिकल मिशन पर न्यूफाउंडलेंड से इंग्लेंड जाते समय प्लेन क्रेश होने के कारण मृत्यु हो गयी। बेंटिग की मृत्यु के बाद बेस्ट ने टोरेन्टो विश्वविद्यालय के बेंटिग और बेस्ट रिसर्च सेन्टर का संचालन पुनः सम्भाला। वहां उन्होंने हिपेरिन पर काफी शोध किया और 1978 में उनकी भी मृत्यु हो गयी।

Contributing to Hindi Wikipedia

Hello, I will try to explain to you how to effectively add information to the hindi wikipedia, without risking deletion. I am writing this message in english since you seem to prefer it for communication.

Firstly, whenever you are writing on a talk page, add 4 tildes(~~~~) at the end of your message. This will automatically produce your signature(your wikipedia username) and you don't have to write your name manually. This is an easy way to identify that you wrote the message.

What I am about to describe below are wikipedia's basic policies on which wikipedia is based. These might be slightly different in wording from the official versions, so in case of any difference the official versions apply.

Verifiability

Wikipedia is an encyclopedia, which is by nature a tertiary source. This means that any information on wikipedia has to come from other sources that can be considered reliable. This includes magazines, newspapers, books and other published material.

This does not include blogs since anyone can write anything on a blog, and hence not everything written on a blog is reliable.

This does not include any internet forums or websites with user generated content. For example: a news story on a newspaper's website is reliable, but comments on it by users are not.

Other notable ubreliable sources include YouTube, any social networking site(like Facebook, Orkut etc.) and if the article is about a person, anything published by the person himself/herself or by any entities owned by the person or affiliated to the person is not considered reliable. For example: If the website of a company abc says that their owner xyz is the best businessman in the world, this information cannot be considered reliable. Only facts may be taken from self-published sources, like a person's name, parents' name, birth date etc. No claims from self-published sources can be considered reliable.

So, if you have to write anything on wikipedia, try to add sources for it using the format <ref>your source's name, publisher etc. </ref>

Note that the tags <ref> and </ref> have to be used. And this entire thing should be placed right after the information that you are using it to reference.

Neutrality

Information on wikipedia has to be neutral. Wikipedia has to ensure that all significant viewpoints regarding a topic are adequately discussed. So, while you may consider a cancer treatment to be highly effective, there may be several people who do not. So we cannot write that xyz is a sureshot way to cure cancer(This is just an example).

Also, any controversial information must be sourced using referencing described above.

No Original Research

Wikipedia cannot contain any original research on anything. This means that we cannot write anything that a reliable source hasn't already said.

Copyrights

All content on wikipedia has to be availaible under a creative commons attribution share-alike 3.0 license. Since the content you posted is not under that license, it cannot be used.

Other Policies

These are other policies that might concern you:

• Wikipedia is not a means to promote your blog. So kindly do not write statements like "अधिक जानकारी के लिये xyz.com पर चटका करें।"
